when you have periodontal disease youre having evasive work done to clean up your mouth. when this work is being done it releases bacteria into your bloodstream, the dental surgeon doesnt want to infect your heart or other organs, especially if you have any kind of heart disease. i have a very minnute case of mitral valve regerge and have to take antibiotics any and everytime i have my teeth cleaned, worked on or even if i have another kind of surgery. your heart is sensitive to this kind of bacteria.

Amoxicillin and metronidazole (flagyl) are a common combination of drugs for treating different periodontal conditions. They have a different spectrum, meaning they target different bacteria, which all are associated with various forms of periodontal disease. Also, other drugs, such as penicillin or clindamycin are given to patients 1 hour prior to dental treatment if they have had problems with their heart. The idea is that periodontal treatment such as scaling, root planing, and any procedure that digs up bacteria can potentially lead to swallowing by the patient. They don't want bacteria to get to your heart, and cause an adverse reaction, like death or secondary infection.
