It generally means that the infection was not adequately treated in the beginning, either because the patient did not take the medication for the full time prescribed, or that the organism causing the infection may have been mis-identified. In either case, follow up treatment is required, with perhaps a new antibiotic.

Some STDs do take that long to show up. That's why a condom should always be used.
