Many hands make light work. All those who love the patient should be involved. This is a big responsibility and requires a lot of hours of care-giving. Find a way that everyone can be involved in looking after him. He will need all the love and support each and everyone can give him. Don't forget to ask the patient what he would like as he should have a say in this as well.

When someone has cancer they need everyone who truly loves them to look after them.

who ever can or is willing to take the best care of them should be the one to take care of them,or who ever the patient wants .now if the parents want to take care of them I'm not sure but the law would probably side with the parents.if your wanting to and he wants you to take care of him and the parents are against it, see a lawyer.

Responsible as in financially, medically caretaking or ?
I would say that its not uncommon for the family to be the primary, unless there was a livein relationship with the gf and then it would probably be her.
There are variables here that are left out that will affect the final answer.

Both, if they want to. A girlfriend might want to be involved, even though it would be pretty hard on her. Parents are always there to help & they should be given that right. As long as they BOTH want to be involved, I don't see why someone should be left out!

If you mean legally, than this would depend on the age of the patient. If the patient is under the age of 18 than his parents are legally responsible. If over the age of 18, than the patient is responsible for himself. He can however ask someone to become a legal guardian at some point in his care.
Personally I would think the parents have more feelings of responsibility towards their 'child' than a girlfriend. There is no legal relationship between a girlfriend and boyfriend so either could end that relationship at any time. Emotionally this might be different . . in either case the girlfriend is still only a girl who is a friend and doesn't have a legal right to be involved. Her involvement would come solely at the wishes of the patient or the parents. Now, if the girlfriend marries the patient . . than she becomes legally responsible at that point.
