What do the numbers mean when talking about the grades of tumors?
Stage 4 means the tumor has metastasized, or spread to other organs or throughout the body. Universally fatal at this point.

In general the higher the number the worse it is. I believe grade four means it has already metastacized, or spread out into the surrounding tissue.
That doesn't mean the person can't be helped, and the type of cancer and the location makes a huge difference.

It's not as simple as everyone is saying. the stages are 1-4 and one is the lowest, 4 is the highest. They stage depeding on where the tumor starts, how far it has metastisized, how much it responds to chemo, as well as the pathology reports after the tumors have been taken out.
I have stage 4 thyroid cancer. But I'm expected to live another 30 years. I have had it a long time, it is an unusual variant that is harder to treat, and it has metastisised to the lymph nodes in my throat and beyond that to the lymph nodes in my chest. Because of all this they classified me as stage 4. However, my doctors are sure that after i get the tumors out and have another dose of radiation, I will continue to get better. Even if it comes back, as long as it's not in a major organ, I will just get it taken out and have another treatment.
Basically the stage 4 cancers are scary, but it depends on where it is as to how "bad or horrific" it is.
If you have stage 4 in your brain or your liver, it's horrific - if it's in your lymph nodes or your skin, it's less so.

All the previous answers are wrong. Folks are confusing grade and stage. Stage 4 is metastatic. However, the grade of tumor could be 1, 2, 3, or 4.
Grade indicates what the cells look like under a microscope ... how normal/abnormal they appear. The higher the grade, the less differentiation there is (the less they look like individual cells). Higher grade is typically more aggressive.

ljkirk13 is the only one who is correct. Grade and stage are two different things and grade is referring to how the cells look. Most cancers are graded from 1-4, 1 meaning the cells look closest to normal. So generally a grade 4 would be very bad and suggest an aggressive cancer. However, it depends on what kind of cancer you are talking about. If it is a prostate cancer which goes up to a grade, or Gleason score, 10 a grade 4 would be mild.
