Many times you don't have obvious symptoms in the early stages of lung cancer. This is one of the reasons why lung cancer is so deadly. It is not found until it is advanced.
Common symptoms are:
persistent cough
shortness of breath
coughing up phlegm, blood
pain when coughing
loss of appetite

To "know" you need a biopsy. Many xrays and even cat scans suggest cancer (when it isn't).
Still - if the chest xray or cat scan suggests lung cancer - it is likely to be present (very likely - especially in smokers).
Symptoms are shortness of breath, pronounced cough, unexplained weight loss, coughing up blood.
