Study sciences and math. Take the most difficult classes in high school and college. Go to a high school for college bound students where you can get the best education and be prepared for college. Then, of course, go to a good college or university.

assuming you've done all the requisites like college and MCATs and are at least starting to apply to medical school... the main thing is that you must WANT to do it. whatever your motivation may be, it really doesn't matter. but getting through all those tests, studying, and then slugging away as an intern and resident really require a lot of hard work. at some point, almost everybody questions their decision to be a doctor. that's really the key: it's not how smart you are; it's how dedicated you are to force yourself to push through those hard times, realizing that the end result is worth it.
